Procedures for referral to early intervention birth to three programs

This form is used to assist staff in completing the process for the referral to early intervention for a child with a disability. All steps of the referral process must also be documented in COPA under child’s case notes.

Delegate Agency Disabilities Enrollment Status Report

This form should be filled out in its entirety and submitted the 10th of each month to the agency’s assigned CPS/DFSS Disability SME and the assigned Education/Disability Support Service Coordinator (SSC).

Joint Screening Referral Consent Form

This form is part of the referral packet that agencies must submit with a child’s referral to Chicago Public Schools for a full and individual evaluation. Milestones

Quarter 4 (Jun - Aug)
  • Review/update delegate agency disabilities service plan.
  • Complete parent orientation on disabilities serves and the EI/CPS referral process.
  • HS/EHS/CCP: Meet 10% disabilities enrollment, as submitted the PIR.
  • Prepare disabilities waiver documents, if required.
